The incidence of acute rejection and graft loss in the first year after renal transplantation (RTx) has decreased since the introduction of potent immunosuppressive protocols. However, chronic allograft nephropathy (CAN) and renal anti-calcineurin toxicity remain important factors leading to graft dysfunction. Several reports have described a correlation between CAN and the number of acute rejection episodes during the first year after RT . Protocol biopsies have been useful for the detection of subclinical rejection and early chronic changes primarily interstitial fibrosis and tubular atrophy. Moreover, protocol biopsies might be of benefit in detecting the signs of anti-calcineurin toxicity which might be important for adapting the immunosuppressive treatment in certain patients. We reported the presence and the frequency of SCR detected by KB in children who were on Tacrolimus (FK) or Cyclosporine (CsA) based protocols.
